I agree to abide by all rules and policies established by the school for the health, safety, and well being of all participants. For the safety and general welfare of all participants, the dance studio reserves the unrestricted right to dismiss a student whose conduct or influence, in the opinion of the director, is contrary to the best interest of the studio. Late entry into a class is at the teacher/director’s discretion. Class placement is at the teacher/ director’s discretion. No street shoes on the studio floors. No gum anywhere in the studios or waiting area. No food/ drinks other than water in the studios. I accept complete responsibility for my child’s and/or my health and well being in this voluntary program. I indemnify and hold harmless, Cynthia King Dance Studio Inc. and its directors, instructors, staff, and volunteers from claims, lawsuits, or any other action or proceeding, which results in or is caused by participation in this program/class. This includes any attorney’s fees or costs in defending any action. I hereby grant to Cynthia King Dance Studio Inc. the irrevocable and unrestricted right to use and publish photographs or video footage of my child for editorial, trade, or advertising in any medium without restriction. I hereby release Cynthia King Dance Studio Inc. and their legal representatives and assigns from all claims and liability relating to said photographs or video footage. I am aware that all payments for dance lessons are non-refundable or transferable. No reduction or allowance will be made for late arrival, early withdrawal, or time missed due to incidental absences including but not limited to illness.
